Job summary

Concordia Visiting Nurses in Canonsburg, PA is seeking a dedicated Registered Nurse (RN) to join our home health team. This role offers the chance to make a meaningful difference in patients' lives, build lasting relationships with families, and work within a supportive, faith-based community.

What you'll do as a Registered Nurse RN:

As a Home Health RN, you'll be at the heart of this mission.

Conduct thorough assessments and developindividualized care plansin collaboration with physicians, patients, and families.

• Treatments, IV starts, Chest Tube Management, and Wound care.

• Educating and empowering patients and familieson managing their health conditions and building confidence in self-care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.

Who you are as a Registered Nurse RN:

• Minimum one year of recent clinical experience as a Registered Nurse recommended.
• Home care experience recommended.
• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.
