Job summary

Concordia Visiting Nurses, part of Concordia Lutheran Ministries, is seeking a Registered Nurse to provide personalized, one-on-one home health care in the Canonsburg area. You will assess patients, develop care plans with physicians and families, and deliver skilled nursing interventions.

Qualifications

• Minimum one year of recent clinical experience as an RN recommended.
• Home care experience recommended.
• Adaptability, compassion, and commitment to patient-centered care.

Responsibilities

• Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients and families.
• Treatments, IV starts, chest tube management, and wound care as applicable.
• Educate and empower patients and families on managing their health conditions and self-care.

As a Home Health RN, you'll be at the heart of this mission.

Conduct thorough assessments and developindividualized care plansin collaboration with physicians, patients, and families.

• Treatments, IV starts, Chest Tube Management, and Wound care.

• Educating and empowering patients and familieson managing their health conditions and building confidence in self-care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
