Job summary

Concordia Lutheran Ministries is seeking a Registered Nurse (RN) to provide compassionate, in-home health care. You will conduct patient assessments, develop individualized care plans with physicians, and deliver direct nursing services in patients' homes.

Ideal candidates have at least 1 year of RN experience (home health preferred), demonstrate strong clinical skills, and can educate families on managing health conditions. Flexibility and autonomous decision-making are valued in this role.

Qualifications

• Minimum 1 year of RN experience (home health preferred).
• Ability to assess and adjust patient care plans in a home setting.
• Strong communication with patients and families.

Responsibilities

• Perform treatments and wound care in patients' homes.

What you’ll do as a Registered Nurse RN:

As a Home Health RN, you'll be at the heart of this mission.

Conduct thorough assessments and developindividualized care plansin collaboration with physicians, patients, and families.

• Treatments, IV starts, Chest Tube Management, and Wound care.

• Educating and empowering patients and familieson managing their health conditions and building confidence in self-care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.

Who you are as a Registered Nurse RN:

• Minimum one year of recent clinical experience as a Registered Nurse recommended.
• Home care experience recommended.
• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.
