Job summary

Concordia Visiting Nurses is seeking a dedicated Registered Nurse to deliver high-quality home health care. You will perform assessments, develop care plans, and collaborate with physicians, patients, and families to optimize outcomes.

This role emphasizes patient-centered care, autonomy, and flexible scheduling within a compassionate team at Concordia Visiting Nurses, part of Concordia Lutheran Ministries and Affiliates.

Qualifications

• Minimum one year of recent clinical experience as a Registered Nurse recommended.
• Home care experience recommended.
• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough assessments and develop individualized care plans with physicians, patients, and families.
• Perform treatments, IV starts, Chest Tube Management, and wound care.
• Educate and empower patients and families on managing health conditions and building confidence in self-care.
• Monitor patient conditions and identify changes using strong clinical assessment skills.
