Job summary
Senior Community Care of Colorado PACE, a program of Volunteers of America, seeks a Nurse Practitioner. This on-site role covers care for elderly PACE participants in Montrose, CO, with an emphasis on comprehensive assessments, care planning, and team collaboration.
The ideal candidate is a registered NP with geriatric experience, prescriptive authority, and a valid driver's license. On-call rotation and cross-site work are required, with strong growth opportunities and competitive compensation.
Qualifications
- Graduate of a school of professional nursing required.
- Licensed as Registered Nurse (R.N.) in the state of employment.
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
- Prescriptive authority required within one year of hire.
- Certified as a Geriatric Nurse Practitioner (G.N.P.) preferred.
Responsibilities
- Provide primary care to PACE participants under Medical Director supervision.
- Perform physical assessments of new participants and semi-annual reassessments.
- Develop and implement care plans for Senior CommUnity Care participants.
- Evaluate and treat chronic and acute conditions in clinic or home settings.
- participate in the on-call rotation and collaborate with IDT for comprehensive care.
