Job summary

Optimal Home Care Inc in Colorado is seeking a Registered Nurse to provide direct patient care, conduct SOC/ROC/RECERTs, and supervise LPNs within assigned accounts.

The role requires a Colorado RN license, at least 1 year of RN experience (home health a plus), and the ability to coordinate care across facilities. Anticipated full-time schedule is 40 hours per week, Mon-Fri, with mileage reimbursement and a comprehensive benefits package.

Qualifications

• Graduate of an accredited nursing program.
• Active Colorado RN license in good standing.
• Minimum 1 year of RN experience; home health a plus.
• Familiarity with OASIS preferred.

Responsibilities

• Perform RN visits, SOC/ROC/RECERTs, and supervisory duties as needed.
• Complete documentation per agency and Medicare guidelines.
• Coordinate care and supervise LPNs in facilities.
• Communicate with facility staff and support census growth.
• Prepare fiscal reports and collaborate with marketers and clinicians.
• Attend care coalitions as required.
• Hours: minimum 40 per week, Mon-Fri.
