Job summary

Optimal Home Care is seeking a Registered Nurse to deliver high-quality home health services to patients in Colorado. The role involves supervising LPNs, performing SOC/ROC/RECERTs, and coordinating care across facilities.

The ideal candidate will hold a Colorado RN license and have at least 1 year of nursing experience, with home health exposure preferred. We offer Mileage reimbursement, competitive benefits, 401K with matching, and a supportive team culture focused on compassionate patient

Qualifications

• Graduate of an accredited, approved school of professional nursing.
• Active Colorado RN license in good standing.
• Must be able to communicate in the language of the client and document client related activity as appropriate.
• Current driver’s license.
• At least one year of experience as a Registered Nurse; Home Health experience a plus.
• Familiarity with OASIS preferred.

Responsibilities

• Complete SOC/ROC/RECERTs, supervisory visits, and revisits as needed (approx 10–12 per week).
• Complete required documentation per agency and Medicare guidelines.
• Provide RN care within scope of practice and supervise LPNs.
• Coordinate care across facilities and accounts; attend resident reviews.
• Communicate with facility staff and contribute to census growth for home health and hospice.
