Job summary

Pride Health is seeking a Home Health RN to support a client facility in Elizabethtown, Kentucky. The role offers a Monday–Friday schedule, 8:00 AM–4:30 PM EST, 40 hours per week, with a 13-week contract and potential extension.

You will perform OASIS assessments, deliver skilled nursing care in the home setting, maintain accurate documentation, and coordinate with physicians and the care team. Pay ranges from $52 to $56 per hour, with mileage reimbursement of 0.48 per mile.

Qualifications

• ADN required for RN role.
• Active Kentucky RN license.
• BLS (AHA) certification.
• Minimum 1 year of RN experience.
• Home health and OASIS experience preferred.

Responsibilities

• Complete OASIS assessments including Start of Care, Resumption of Care, Recertification, and Discharge visits.
• Deliver skilled nursing care and ongoing patient assessments in the home setting.
• Maintain accurate and timely clinical documentation.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Participate in on-call rotation and ensure high-quality patient care.

Job Details

Job Type: Available for both Local and Travel

Contract Length: 13 Weeks with a high possibility of extension

*Pay offered is based on experience, expertise, credentialing, and education.
