Job summary

Pride Health is seeking a Registered Nurse - Home Health to support our client’s healthcare facility in Albuquerque, NM. This full-time role involves local travel and a 14-week contract with pay range $50.00 to $55.00 per hour, based on experience and credentials.

Responsibilities include performing SOC/POC, OASIS assessments, and coordinating care with physicians and therapists while delivering skilled nursing interventions in the home setting.

Qualifications

• RN license in New Mexico (active)
• Associate Degree in Nursing (ADN) or Nursing Diploma required
• Minimum 1–2 years RN clinical experience
• Minimum 2 years Home Health experience
• Homecare Homebase (HCHB) experience required
• OASIS assessment experience required

• Perform comprehensive home health assessments and identify patient needs
• Develop, implement, and evaluate Plans of Care based on physician orders
• Complete SOC, ROC, Recertification, Discharge, and OASIS assessments
• Coordinate care with physicians, therapists, home health aides, caregivers, and referrals
• Deliver skilled nursing interventions including wound care, venipuncture, IV therapy, catheter care, medications
• Assess and manage pain and educate patients and families on symptom control

Job Details

Schedule: Full-Time | 8 Hours Per Day (40 Hours Per Week)

Location: Albuquerque, NM 87109

Float / Coverage Area: Local Travel Required

Job Type: [Local contract / Permanent/ Per Diem] : Local Contract

Contract Length: 14 Weeks

Pay Range: $50.00 to $55.00/hour*

Mileage Reimbursement Rate (if applicable) : $0.50/ mile

Pay offered is based on experience, expertise, credentialing, and education.

Duties

• Perform comprehensive home health assessments in both routine and emergent situations to identify physical, psychosocial, environmental, spiritual, and cultural patient needs.
• Develop, implement, revise, and evaluate individualized Plans of Care (POC) based on patient needs and physician orders.
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and OASIS assessments.
• Coordinate care with physicians, therapists, home health aides, caregivers, referral sources, and other healthcare professionals.
• Deliver skilled nursing interventions including wound care, venipuncture, IV therapy, catheter care, medication administration, respiratory treatments, and disease management.
• Assess and manage pain using appropriate pain management techniques while educating patients and families regarding symptom control.

Requirements

• Active Registered Nurse (RN) License in the State of New Mexico.
• Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma Program.
• Minimum 1-2 years of RN clinical experience.
• Minimum 2 years of Home Health experience.
• Homecare Homebase (HCHB) experience required.
• OASIS assessment experience required.
