Qualifications

• Graduate of an accredited nursing school.
• Associate degree required; BSN preferred.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.

Responsibilities

• Implements and monitors patient care plans.
• Serves as primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assess discharge planning needs with the healthcare team.
• Provide individualized care addressing physical, psychosocial, cultural, spiritual and age-related needs.
• Delegate tasks to nursing staff to maximize performance.
• Participate in daily interdisciplinary patient rounds.
• Align with ANA standards and ethics.
