Qualifications

Responsibilities

• Implements and monitors patient care plans and communicates patient condition.
• Serves as primary coordinator of all disciplines for well-coordinated care.
• Notes and carries out physician and nursing orders.
• Assesses discharge planning needs with the healthcare team.
• Delivers individualized care based on patient needs and age-related considerations.
• Delegates tasks to nursing staff to meet patient needs and maximize performance.
• Participates in daily interdisciplinary patient rounds and provides input.
• Adheres to ANA standards and ethics statements.
• Develops and revises individualized care plans.
