Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to deliver comprehensive, patient-centered care across biological, physical, behavioral, social, and nursing sciences in home-care settings.

You will assess, plan, implement, and evaluate care, document in EMR, mentor others, and pursue ongoing professional growth. The role requires current licensure and CPR/BLS, with ACLS anticipated in some units.

Qualifications

• Current Ohio RN licensure and BLS certification; ACLS within 6 months for specific units.
• All hires after Jan 2010 must obtain a BSN within five years; MSN or DNP preferred but not required.
• Participation in professional organizations and continuing education is encouraged.

Responsibilities

• Assess holistic physical, psychosocial, spiritual, and educational needs; identify abnormal data and safety risks.
• Identify problems, develop care plans with measurable goals, and coordinate discharge planning.
• Deliver evidence-based care, delegate tasks, educate patients and families, respond to emergencies, ensure safety.

Qualifications include:
