Qualifications

Responsibilities

• Assess patients: Evaluate physical, emotional, spiritual, and educational needs with the healthcare team.
• Develop care plans with patients and families to meet needs and goals.
• Deliver patient care with evidence-based practices and patient education.
• Educate patients and families and document care per hospital policies.
• Evaluate outcomes and adjust care based on evidence and results.
