Job summary

Qualifications

• Current New York State Registered Nurse (RN) license in good standing.
• 1+ year of nursing experience.
• Experience with individuals with developmental disabilities in residential or community settings preferred.
• Strong clinical assessment skills to identify and respond to medical conditions.
• Ability to educate and train staff on health topics, infection control, and emergency protocols.
• Strong organizational and documentation skills; support OPWDD standards.

Responsibilities

• Assess medical conditions of persons supported to develop targeted care plans.
• Conduct health assessments and nursing reviews; administer medications per prescriptions.
• Coordinate with physicians, DPSs, families, and other healthcare providers.
• Provide telephone triage and consultation for health concerns.
• Maintain accurate nursing documentation; ensure regulatory compliance.
• Train and oversee unlicensed staff in medication administration and safety.
