Job summary

The New York Foundling in New York, NY is seeking a Registered Nurse to provide high-quality, person-centered medical care within our Developmental Disabilities Division. You will assess health needs, plan care, coordinate medical services, and support staff to ensure safety and well-being.

Required: NYS RN license in good standing, 1+ year nursing experience, experience with individuals with developmental disabilities, BLS certification, and strong documentation.

Qualifications

• Current New York State Registered Nurse (RN) license in good standing.
• Minimum of 1 year nursing experience.
• Experience with individuals with developmental disabilities preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Assess persons supported medical conditions to develop targeted care plans and improve health outcomes.
• Conduct regular health assessments and nursing reviews; administer meds as prescribed and monitor effects.
• Coordinate with physicians, DSPs, families and other providers to align care efforts.
• Provide telephone triage and health consultations for urgent concerns.
• Work with the care team to reduce avoidable hospitalizations or ER visits.
• Maintain thorough documentation and ensure regulatory compliance (HIPAA, OPWDD, CMS).
• Train and oversee unlicensed staff in medication administration and health monitoring.
