Qualifications

• Current New York State RN license (required).
• 1+ years nursing experience.
• Experience with developmental disabilities preferred.
• Strong clinical assessment skills to identify and respond to health changes.
• Ability to educate and train staff on health topics and emergency protocols.
• Strong organizational and documentation skills for health records.

Responsibilities

• Assess patients' medical conditions to develop targeted care plans and promote long-term well-being.
• Conduct health assessments and nursing reviews; administer medications and monitor for adverse reactions.
• Coordinate with physicians, families and other providers to align care and reduce gaps in medical management.
• Provide telephone triage and consultation to persons supported and staff for urgent health concerns.
• Train and supervise unlicensed staff in medication administration and health monitoring.
• Maintain thorough documentation and ensure compliance with regulations (e.g., Medicaid, CMS, HIPAA).
• Oversee medication safety and contribute to audits and quality assurance initiatives.
