Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver compassionate, in-home hospice care. You will lead individualized care plans, conduct assessments, and coordinate a care team to support patients and families with dignity.

Qualifications

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of recent acute care experience required; hospice/home health preferred.
• Ability to complete comprehensive assessments and develop plans of care.

Responsibilities

• Lead a care team to ensure symptom management with empathy.
• Administer medications and treatments as prescribed.
