Job summary

St. Croix Hospice is seeking an RN Case Manager to lead in-home hospice care, partnering with a care team to deliver personalized plans and compassionate symptom management.

You will conduct comprehensive assessments, educate families, and ensure high-quality end-of-life care with empathy and clinical excellence. The role requires an RN license and 1+ year acute care experience, with on-call duties and a reliable vehicle.

Qualifications

• RN licensure in good standing in the United States.
• 1+ years of acute care experience; hospice/home health preferred.
• Complete initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with patient and family.
• Educate patients and caregivers on care techniques and medications.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities
