Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1, whole-person home care. You will coordinate individualized care plans with patients and families, educate caregivers, and support complex decisions with professionalism and empathy.

Requirements include an accredited nursing degree and 1+ years of acute care; hospice/home health experience is preferred. Must have reliable transportation and a valid driver's license. Full-time schedule with on-call duties.

Qualifications

• RN with an accredited nursing program (ASN/BSN/MSN).
• 1+ years of acute care experience required; hospice/home health preferred.
• Ability to educate patients and families about care planning and symptom management.
• Reliable transportation and valid driver’s license required.

Responsibilities

• Perform initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Coordinate with an integrated care team and ensure seamless care transitions.
• Rotate on-call duties as part of on-call schedule.
