Job summary

St. Croix Hospice in Iowa is seeking a Registered Nurse Case Manager to deliver 1:1 holistic care to patients and families in their homes, coordinating with an interdisciplinary team to develop individualized care plans and guide end-of-life conversations with compassion.

You will administer medications and treatments, educate families on symptom management, rotate on-call duties, and ensure smooth care transitions, requiring reliable transportation and an active RN license.

Qualifications

• 1+ years of acute care experience required; hospice/home health experience preferred.
• RN license in good standing is required to practice as a hospice case manager.

Responsibilities

• Provide 1:1 holistic support to patients and families in their homes.
• Develop individualized care plans with the patient, family, and interdisciplinary team.
• Guide end-of-life conversations with compassion and professionalism.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless transitions of care.
