Job summary

At St. Croix Hospice, we provide compassionate end-of-life care in patients' homes across the Omaha area.

As a Registered Nurse Case Manager, you’ll deliver 1:1, whole-person care and coordinate with an integrated care team to build and adjust personalized plans for patients and their families. You will conduct assessments, educate caregivers, administer medications, rotate on-call duties, and ensure smooth transitions between care settings.

Qualifications

• Active RN license in good standing.
• 1+ years of acute care experience; hospice/home health preferred.
• Ability to develop individualized care plans with patients and families.
• Strong communication, empathy, and professional conduct.

Responsibilities

• Perform initial and ongoing assessments to determine hospice needs.
• Develop and update care plans with patient/family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and coordinate care transitions.
