Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to lead 1:1 home-based care, coordinating with an interdisciplinary team to plan and adjust hospice services.

You will conduct assessments, develop care plans, educate families, and manage medications, while ensuring comfort, dignity, and support in patients' homes. On-call duties are required. This is a full-time role with comprehensive benefits.

Qualifications

• Must hold an active RN license and have completed an accredited nursing program.
• 1+ years of acute care, hospice/home health experience preferred.
• Ability to administer medications and educate patients and caregivers.
• Reliable transportation and valid driver’s license required.

Responsibilities

• Perform initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with the patient and family.
• Provide 1:1 holistic home-based nursing care.
• Rotate on-call duties and ensure seamless care transitions.
