The Care Team Hospice in Barberton and surrounding areas is seeking a compassionate RN Case Manager to coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers and the interdisciplinary team to ensure comfort and quality of life for each patient. This full‑time role runs Monday–Friday, with on‑call opportunities and compensation.
