The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Ohio. You will provide skilled nursing care, conduct assessments, and manage symptoms in accordance with individualized care plans, while building meaningful relationships with patients and families.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours on-call situations.
• Collaborate with physicians and interdisciplinary team to coordinate care and outcomes.
• Educate patients and families on symptom management, disease progression, medications, and end-of-life care.

The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Case Manager to join our team. This is a rewarding opportunity for a nurse who is passionate about providing exceptional end-of-life care while building meaningful relationships with patients and families.

Key Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after‑hours and on‑call situations, to ensure exceptional patient‑centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end‑of‑life care while providing compassionate support.

Schedule & Work‑Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On‑call opportunities available with additional compensation
• Some locations offer dedicated on‑call teams, resulting in backup coverage only
