Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager for Barberton and surrounding areas. You will coordinate patient care, manage plans of care, and work with physicians, caregivers, and the interdisciplinary team to ensure comfort and dignity at end-of-life.

Active RN license is required; hospice nursing experience is preferred. The role is full-time, in-person, with on-call opportunities and a strong benefits package.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and clinical interventions with timely documentation in HCHB.
• Respond to patient/family needs, including after-hours calls, to ensure patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.
