The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Akron, OH. This role focuses on coordinating patient care, managing individualized care plans, and collaborating with physicians, families, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management and end-of-life care.
