Qualifications

• Bachelor's Degree in Nursing or Social Work; RN preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• CCM certification is a plus.
• Must maintain continuing education/licensing and stay in good standing with the Department of Public Health.

Responsibilities

• Communicate with residents and family about Plans of Care and treatment protocols.
• Attend morning meetings to facilitate team communications.
• Answer residents' questions about care and illness progression.
• Monitor and adjust residents' statuses based on changing needs.
• Organize daily work by assessing resident needs and priorities.
• Complete required documentation.
• Perform utilization review to ensure cost-effective care.
• Coordinate care with residents, providers, and payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Maintain professional interactions with residents, families, staff and partners.
• Accurate charting and timely paperwork.
• Work independently with sound judgement in interactions with physicians and payers.

Education

Bachelor's Degree in Nursing or Social Work

RN license preferred

LPN/LVN license acceptable

Two years of clinical nursing experience preferred

Medicare/Medi-Cal/Medicaid knowledge

CCM certification a plus

Continuing education/licensing

Good standing with Dept. of Public Health
