Job summary

UnitedHealth Group is seeking a Clinic Case Manager in Las Vegas to support high-risk patients. You will work with a multidisciplinary team to educate, advocate, and coordinate care, aiming to maximize quality of life for patients and families.

You will operate in a less structured environment under RN license scope, ensuring compliance with policies and continued training. The role emphasizes care transitions, performance on quality measures, and collaboration across care teams.

Qualifications

• NV RN License is required and BLS certification is expected within 3 months.
• Minimum 2+ years of acute care experience and nursing assessment skills.
• Proficient with MS Office and electronic medical records (EMR).
• Strong communication, interpersonal and customer service abilities.
• Ability to multi-task and work under pressure with sound judgment.
• Valid NV driver's license and reliable transportation.

Responsibilities

• Embedded in the primary care clinic, collaborating with clinical and non-clinical teams.
• Assess patients, coordinate with caregivers and clinicians to address needs.
• Implement interventions to reduce ER visits and hospitalizations.
• Facilitate transition of care from ED/inpatient to outpatient, including referrals.
• Achieve quality measures outcomes by reducing gaps in care.
