Job summary

Medical University of South Carolina in Charleston, SC is seeking a Nurse Case Manager I (PRN) to coordinate an interdisciplinary plan of care from admission to discharge. You will liaise with patients, families, clinicians, and insurers to optimize outcomes and facilitate timely transition.

Required is a BSN, SC or compact RN license, and 3 years of RN experience; leadership and case management/utilization review background preferred. BLS/ACLS certification is required.

Qualifications

• Bachelor's degree in Nursing from an accredited school of nursing and three years of RN experience is required.
• Licensure as a registered nurse in SC or compact state is required.
• BLS or ACLS certification required (AHA or ARC) for professional rescuer/healthcare provider.

Responsibilities

• Coordinate interdisciplinary plan of care from admission to discharge.
• Act as liaison between patient/family, healthcare staff, and insurers.
• Develop discharge plans using available resources to meet ongoing patient needs.
