Job summary

First Light Home Care is seeking an RN Care Manager to lead in-home client assessments, develop person-centered care plans, and mentor caregivers in North Brunswick. You will collaborate with physicians, therapists, and schedulers to maintain safe, high-quality home-based care for seniors.

Ideal candidates hold an active RN license, preferably with a Bachelor’s in Nursing or related field, and have strong clinical assessment skills, CPR/First Aid certification, and a valid driver’s license.

• Bachelor’s in Nursing or related human-services field, or 2+ years geriatric/home-care experience.
• Strong clinical assessment skills and problem-solving abilities.
• Warm, professional communication with families and field staff.
• Valid driver’s license.

Responsibilities

• Assess and plan in-home care needs; craft individualized care plans with client/family input.
• Coach and supervise caregivers; ensure field competency and adherence to plans.
• Monitor vitals, conditions changes, and interventions in HIPAA-secure records.

• Assess & Plan
  • Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Coach & Supervise
  • Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Monitor & Document
  • Track vitals, condition changes, and interventions in our HIPAA-secure EMR; elevate concerns promptly.
• Collaborate & Advocate

What You’ll Bring
