Job summary

First Light Home Care is seeking an RN Care Manager to design person-centered care plans and mentor caregivers in North Brunswick. You’ll work autonomously with support from the office and families.

Qualifications

• Active RN license in good standing.
• Bachelor’s in Nursing or related human-services field, or 2+ years’ geriatric/home-care experience.
• CPR/First Aid certification and valid driver's license.
• Strong clinical assessment skills and problem-solving abilities.
• Warm, professional communication with families and field staff.

Responsibilities

• Design person-centered care plans with client/family approval.
• Mentor and orient caregivers to each care plan.
• Perform field competency checks and ensure ongoing compliance.
• Track vitals and document interventions in HIPAA-compliant EMR.
• Coordinate with physicians, therapists, and schedulers to keep care seamless.

  • Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Coach & Supervise
  • Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Monitor & Document
  • Track vitals, condition changes, and interventions in our HIPAA-secure EMR; elevate concerns promptly.
• Collaborate & Advocate

What You’ll Bring

• Active Registered Nurse (RN) license
