How You’ll Make an Impact

Assess & Plan

Completed in‑home RN assessments; craft individualized Care Plans with client/family approval.

Coach & Supervise

Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.

Monitor & Document

Track vitals, condition changes, and interventions in our HIPAA‑secure EMR; raise concerns promptly.

Collaborate & Advocate

Coordinate with physicians, therapists, and schedulers to keep care seamless and client‑centered.

Quality & Compliance

Maintain audit‑ready client files, collect initial service payments, and uphold state home‑care regulations.

What You’ll Bring

• Active Registered Nurse (RN) license in New Jersey
• Bachelor’s in Nursing or related human‑services field – and– 2+ years’ geriatric/home‑care experience
• Solid clinical assessment skills and a passion for problem‑solving
• CPR/First Aid certification and a valid driver’s license
