Job summary

Essentia Health is seeking a registered nurse to provide triage and health advice by phone for patients and clients. You will use evidence-based protocols to assess symptoms and determine disposition, while documenting every call in real time.

Responsibilities include processing prescription refills, coordinating transfers, and supporting transitional care for discharged patients. An evening shift and possible remote/hybrid options are available; RN licensure in MN/ND/WI is required or

Qualifications

• RN license in MN, ND or WI upon hire
• Ability to obtain RN license in other states within 3 months
• Evening shift from 1pm to 4pm; remote/hybrid option available
• FTE: 0.8 to 1.0 depending on schedule

Building Location:Business Service CenterDepartment:1004220 ESSENTIA NURSE CARE LINE - EH SSJob Description:Provides triage and health advice by phone to patients, consumers, and clients. Utilizes computerized Barton Schmitt /David Thompson evidence based triage protocols to assist in assessing patients’ symptoms to arrive at a patient disposition. Also receives requests for information on medical conditions, medications, diagnostic tests, and prescription refills. All calls are documented electronically in real time. In addition, responsibilities include processing prescription refill requests received from pharmacies and patients, utilizing the Essentia Health primary care refill protocol. Will assist with the coordination of patient transfers from referring facilities calling in to the Essentia Health STAT doc line. May also participate in transitional care calls to patients who have been discharged from an Essentia Health hospital. Works with ambulatory computer software systems.
