Registered Nurse (RN) – Care Coordination | Bilingual Spanish

Are you a Spanish-speaking Registered Nurse with a Med-Surg, Telemetry, or other clinical nursing background looking for an opportunity outside of traditional bedside nursing?

A community-based healthcare organization in Queens is seeking a Bilingual Registered Nurse (RN) to join its clinical team in a care coordination-focused position. This is an opportunity to continue using your nursing experience and clinical judgment while focusing on patient education, care planning, follow-up, and helping patients navigate their healthcare needs.

Med-Surg and Telemetry RNs are encouraged to apply. Previous care coordination experience is not required.

Schedule & Compensation

• $100,000 annual salary
• Full-time
• Monday–Friday
• On-site in Queens

What You'll Do

In this role, you'll work closely with patients, providers, and other members of the healthcare team to help patients receive appropriate and consistent care.

Responsibilities include:

• Provide ongoing clinical support and guidance to patients
• Communicate with patients regarding their healthcare needs and treatment recommendations
• Help patients understand their care plans and next steps
• Identify patients who may benefit from additional clinical support or follow-up
• Assist patients in identifying challenges that may impact their ability to receive care
• Coordinate services and follow-up across members of the healthcare team
• Work closely with providers to support individualized patient needs
• Provide patient education and reinforce treatment recommendations
• Conduct patient outreach and follow-up as appropriate
• Maintain accurate and timely clinical documentation
• Collaborate with clinical and administrative team members to support continuity of care
• Contribute to initiatives focused on improving patient care and outcomes

What We're Looking For

• Active and unrestricted New York State Registered Nurse (RN) license
• Med-Surg or Telemetry experience is highly desirable
• Strong clinical judgment and patient communication skills
• Comfortable educating and communicating with patients
• Strong organizational and documentation skills
• Ability to collaborate effectively with multidisciplinary healthcare teams

Experience in ambulatory care, primary care, community health, case management, or care coordination is helpful but not required.
