Transitions Hospice is seeking an experienced RN for the night shift to provide comprehensive hospice nursing care in a home setting in Illinois. You will conduct assessments, develop and update care plans, administer medications, and document outcomes while coordinating with physicians and family members.

The role requires an active RN license, bedside clinical judgment, and the ability to work independently during night hours.

Qualifikationen

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Comfort and composure working independently during overnight hours.
• Strong assessment skills and sound clinical judgment in high-pressure moments.
• Excellent documentation habits and communication skills.
• Valid driver’s license and willingness to travel locally.

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct thorough patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet overnight patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health data and respond with clinical judgment.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support continuity of care.
• Ensure equipment and necessary services are in place for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.

Description

Coverage Area: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas

Salary: $80,000 - $90,000 / year

Schedule: 8 PM–8 AM, alternating 4-day and 3-day weeks including weekends in rotation
