Job summary

UnitedHealth Group is seeking a Registered Nurse for an After Hour On-Call role in hospice/home health. The position includes on-call shifts Friday through Sunday from 5p to 8a, focusing on triage, assessment, and coordination to ensure timely, quality care for patients at home.

Candidates must hold an active RN license for the applicable state, CPR certification, at least 1 year of nursing experience in hospice/home health or acute care, and a valid driver’s license.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.

• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.

Registered Nurse - Hospice After Hour On-Call RN

On-Call Schedule: Friday, Saturday and Sunday 5p-8a

Primary Responsibilities:

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans

Maintain accurate and timely documentation of all patient interactions and clinical decisions

Required Qualifications:

Current RN license, specific to the state(s) you are assigned to work.

• Current CPR certification.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Valid driver's license, reliable transportation, and liability insurance.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $90,650.00 to $135,976.00 annually based on full-time employment. We comply with all minimum wage laws as applicable.
