Job summary

UnitedHealth Group is seeking a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care to ensure continuity and quality outcomes. This role blends remote and field-based nursing to support patients at home and in community settings.

You will assess conditions by phone or in-person, collaborate with physicians and care teams, educate patients and caregivers on symptom management, and document all clinical interactions accurately.

Qualifications

• Current RN license, specific to the state you are assigned to work.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

As the Hospice RN On-Call, you are responsible for providing after-hours clinical support, triaging patient needs, and coordinating care to ensure continuity and quality outcomes. This role delivers remote and field-based nursing support, assesses patient conditions, and collaborates with interdisciplinary teams to address urgent and emergent patient needs.

Primary Responsibilities:

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions

Required Qualifications:

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $79,600 to $119,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
