Job summary

Amedisys, part of the Optum family, is seeking a Hospice RN On-Call to provide after-hours clinical support across remote and field settings, ensuring continuity and quality outcomes. You will triage patient needs, coordinate with care teams, and educate patients and caregivers while documenting every interaction. Schedule: Monday – Thursday 5pm–8am.

$7,500 sign-on bonus. Requires active RN license in the assigned state(s), CPR, and 1+ years in hospice/home health or acute care.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.

As the Hospice RN On-Call, you are responsible for providing after-hours clinical support, triaging patient needs and coordinating care to ensure continuity and quality outcomes. This role delivers remote and field-based nursing support, assesses patient conditions and collaborates with interdisciplinary teams to address urgent and emergent patient needs.

Schedule: Monday - Thursday 5p - 8a

Primary Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions

Required Qualifications

• Current RN license, specific to the state(s) you are assigned to work
• Current CPR certification
• 1+ years of nursing experience, preferably in hospice, home health, or acute care
• Valid driver's license, reliable transportation, and liability insurance

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $79,600 to $119,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
