The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Case Manager to join our team in Canton, OH. This role centers on coordinating patient care and managing individualized plans of care within a supportive team.

You will collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and their families. On-call opportunities may be available with additional compensation.

Qualifications

• Active RN license in Ohio.
• Hospice or palliative care experience preferred.
• Ability to coordinate with physicians and families.
• Strong communication and compassionate bedside manner.

• Covering Canton Areas
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
