Job summary

Access Home Health is seeking a Registered Nurse to join our Home Health team in Mobile, Alabama. The RN will provide and direct nursing care in patients' homes, coordinating with physicians and family members to maintain continuity of care.

Responsibilities include evaluating patients, developing plans of care, and ensuring compliance with state guidelines, with responsibilities spanning admit through discharge. A strong emphasis on quality care and efficient visit routing is expected.

Qualifications

• Active RN license in Alabama.
• Current CPR certification.

A few position highlights include:Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
