Job summary

Home Care Solutions is seeking a Registered Nurse to join our Home Health team in Dickson, TN. The RN will coordinate the plan of care, perform initial evaluations, and ensure quality documentation from admission to discharge.

Requirements include 1 year of RN experience, a current RN license in the state of service, CPR certification, and access to reliable transportation for daily travel to patient residences.

Qualifications

• 1 year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Coordinate the total plan of care with other health care professionals to maintain continuity of patient care.
• Perform initial and/or comprehensive nursing evaluation visits and develop the plan of care within state guidelines.
• Maintain and submit accurate documentation and clinical notes of patient condition, goals, interventions and responses to care.
• Provide high quality nursing services within the scope of practice per state laws and plan of care, from admit to discharge.
