Access Home Health is seeking a Registered Nurse to join our Home Health team in Mobile, Alabama. The RN will provide and direct nursing care in patients' homes, coordinating with physicians and family members to maintain continuity of care.

Responsibilities include evaluating patients, developing plans of care, and ensuring compliance with state guidelines, with responsibilities spanning admit through discharge. A strong emphasis on quality care and efficient visit routing is expected.

Qualifications

• Active RN license in Alabama.
• Current CPR certification.

Responsibilities

• Create, implement, and coordinate the plan of care for home health patients.
• Conduct initial/comprehensive nursing evaluations and determine primary focus of care.
• Maintain documentation and clinical notes including patient progress and outcomes.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Deliver high quality clinical services within state nursing scope of practice.
