Job summary

The University of Tennessee is seeking a Registered Nurse to join our Home Health team in Maryville, TN. The RN will provide and direct nursing care to patients in their homes, coordinating with physicians and families to optimize wellness and reduce rehospitalization.

Responsibilities include initial assessments, plan of care development, accurate documentation, and delivering high-quality care across the patient journey from admit to discharge.

Qualifications

  • RN license active in the state where services are rendered.
  • Current CPR certification and valid driver's license.
  • Access to reliable transportation for daily patient visits.
  • Proven experience in home health or similar settings.

Responsibilities

  • Create, implement, and coordinate the plan of care to promote wellness and prevent rehospitalization.
  • Conduct initial/comprehensive nursing evaluation and determine focus of care within guidelines.
  • Maintain accurate documentation and clinical notes on patient status and outcomes.
  • Establish an efficient visit route and deliver care from admission to discharge.
  • Provide high-quality nursing care within state scope and agency policy.

A few position highlights include:Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
