Job summary

Qualifications

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Must have a valid driver’s license, vehicle insurance, and access to dependable transportation to visit multiple patient residences daily.

• Create, implement, and coordinate the plan of care to promote wellness and prevent re-hospitalization.

A few position highlights include:Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
