Job summary

Interim HealthCare of Northern Indiana is seeking a Home Health Registered Nurse (RN) to provide in-home care to patients throughout South Bend and surrounding areas. The role includes coordinating with physicians and supervising LPNs and aides, performing assessments, documenting progress, and educating patients on their plan of care and safety.

Qualified candidates hold an active Indiana RN license, have at least one year of nursing experience (preferably home health), CPR certification, and

• Graduate of an accredited nursing program with active Indiana RN license.
• Minimum of 1 year of nursing experience, preferably in home health.
• CPR certification.
• Strong communication and compassionate care.

Responsibilities

• Provide home-based care to patients in their homes.
• Coordinate with physicians and supervise LPNs, CNAs and aides.
• Perform assessments, document progress, and report changes to the physician.
• Educate patients and families on plan of care, medications, therapy, and safety.
• Coordinate discharge from services.
