Job summary

Interim HealthCare Inc. is seeking a Home Health Registered Nurse to advocate for patients, coordinate care with physicians, and guide families on plans of care. You will operate within a patient-centered team and manage care during and after hospital or facility discharge.

The role features a Monday–Friday 8am-5pm schedule, 1:1 nurse-to-patient ratios, a company car, and comprehensive benefits including PTO, holiday pay, and health, dental, and vision insurance, plus 401K and profit sharing.

Qualifications

• Graduate of an accredited nursing program and active RN license in Ohio.
• Minimum of 1 year nursing experience.
• CPR certification.
• Clinical proficiency, strong communication, and compassion.
• Home Health experience preferred but not required.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases after discharge.
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress and report changes to the physician.
• Educate patients on plan of care, prescribed medication, therapy, diet and exercise.
• Coach family members on the plan of care, medication and home safety.
• Ensure goals are met and coordinate discharge from services.
• Typically has a caseload of 30-35 patients and 5-7 visits per day.

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient’s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services
• Typically has a case load of 30-35 patients
• Assigned visits per day based on a point system which averages 5-7 visits per day

A few must-haves for Home Health Registered Nurses:

• Graduate of an accredited nursing program and active RN license in Ohio
• Minimum of 1 year of nursing experience
• CPR certification
• Clinical proficiency in nursing specialty, strong communication skills and compassion
• Home Health experience preferred but not required.
