Job summary

Ascension Health in Mequon, WI is seeking a Part-Time Registered Nurse to lead personalized care plans and coordinate a multidisciplinary team in the Med Surg department.

The role focuses on translating physician orders into bedside actions with a strong emphasis on safety and patient outcomes.

Qualifications

• BLS Provider within 1 month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Responsibilities

• Lead the implementation of personalized care plans, continuously monitoring patient progress to ensure every individual receives the right intervention at the right time.
• Coordinate a multidisciplinary team of specialists to provide a seamless, holistic care experience that prioritizes the patient's clinical and emotional needs.
• Translate complex physician orders into compassionate bedside actions, maintaining the highest standards of safety and clinical excellence.
• Advocate for patients throughout their recovery journey, documenting every step of the care process to ensure clear communication and high-quality outcomes.
• Facilitate comprehensive discharge planning with the healthcare team to ensure patients and their families feel confident and supported as they transition home.

Education

Diploma from an accredited school/college of nursing

Licensure from Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State

What minimum requirements you’ll need

Licensure / Certification / Registration
