The Avenue at Lyndhurst is seeking a registered nurse to join our post-hospital rehabilitation and skilled nursing care team in Lyndhurst, OH. You will provide direct patient care, administer medications, manage wounds and nutrition, and work with a multidisciplinary team to implement care plans.
Requirements include current Ohio RN registration, active license in good standing, and a minimum of 2 years in skilled nursing/long-term care.

As a Registered Nurse at The Avenue at Lyndhurst, you will have the opportunity to work with a diverse patient population, providing high-quality care in a state-of-the-art facility. You will be responsible for:
Providing direct patient care, including administering medications, wound care, and nutrition management
Developing and implementing individualized care plans in collaboration with the interdisciplinary team
Conducting patient assessments, monitoring vital signs, and reporting changes to the healthcare team
Collaborating with therapists and other healthcare professionals to ensure a comprehensive approach to patient care

Requirements:
Current registration as a Registered Nurse in the state of Ohio
Active RN license in good standing
Minimum 2 years of experience in a skilled nursing/long-term care facility is a must!
