Job summary

The Avenue at Lyndhurst is seeking a registered nurse to join our post-hospital rehabilitation and skilled nursing care team in Lyndhurst, OH. You will provide direct patient care, administer medications, manage wounds and nutrition, and work with a multidisciplinary team to implement care plans.

Requirements include current Ohio RN registration, active license in good standing, and a minimum of 2 years in skilled nursing/long-term care.

Qualifications

• Current registration as a Registered Nurse in the state of Ohio.
• Active RN license in good standing.
• Minimum 2 years of experience in a skilled nursing/long-term care facility is a must!

Responsibilities

• Providing direct patient care, including administering medications, wound care, and nutrition management.
• Developing and implementing individualized care plans in collaboration with the interdisciplinary team.
• Conducting patient assessments, monitoring vital signs, and reporting changes to the healthcare team.
• Collaborating with therapists and other healthcare professionals to ensure a comprehensive approach to patient care
