Innovativ Pharma, Inc. is seeking a Registered Nurse (Dialysis) to lead and coordinate nursing care, training, and education. The role directs LPN/LVN and PCT staff and collaborates with an interdisciplinary team to improve patient outcomes.
The position requires an active state RN license, BLS certification, and dialysis experience. Occasional travel may be needed, with emphasis on patient advocacy and adherence to safety standards.

The Registered Nurse (dialysis) is responsible for coordinating and providing the nursing care, training, and education. The Registered Nurse works collaboratively with team members, participates in program development. This position provides direction to the LPN/LVN and PCT positions.
Job Description/Duties:
Collects, analyzes and records relevant patient health data accurately
Analyzes assessment data to identify patient problems
Develops a plan of care that describes interventions to attain expected outcomes
Implements and records interventions to attain expected outcomes in EMR
Evaluates patient progress toward attainment of identified goals
Adheres to approved, research-based practice standards
Directs and supervises patient care provided by LPN/LVNs and support personnel
Utilizes equipment and materials in a safe and cost effective manner
Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures
Functions as patient advocate and ensures care is delivered in a non-judgmental and non-discriminatory manner
Assures patients are informed consent and have signed for indicated procedures
Consistently participates as an active member of department performance improvement committee
Responsible for QAPI tracking and action plans to meet goals. Meets with the Nurse Manager as needed to drive QAPI goals
Uses current data and research in order to have the home program reflect industry trends that will improve patient outcomes
Works collaboratively with the interdisciplinary team, other departments and outside institutions
Works collaboratively with team members to maintain inventory
Supports dialysis teaching day and assists with planning
Provides nursing care, training, education
Provides support for home patients and family members while maintaining professional boundaries
Provides direct patient care as needed in response to on-call issues
Collaborates with MD and staff on patient care as needed
Maintains clinical staff competencies
Ensures all required regulatory training attained annually
Performs similar or related duties as requested or directed
BS in Nursing preferred
AA or Diploma from School of Nursing
At least 12 months Hemodialysis experienced (18 months in Texas)
Assess patient status and take appropriate action
Able to function at a high level with little operational oversight
Exemplary interpersonal skills; able to effectively communicate (verbal and written) with diverse backgrounds and different organizational levels
Able to define problems and present solutions
Valid State Nursing License required
Current CNN or CDN required or must be obtained within one year from date of hire
Current BLS certification required
Current driver’s license
Able to sit and work at computer for extended periods of time
Walk, sit, lift no more than 50 pounds
Must be able to identify and differentiate colors
Must be willing to travel to network facilities and sites as needed
